As I understand, Dr. Sartor will make a detailed assessment of your situation and then tailor a treatment plan for you as an individual. There is no standard first step in all cases.
But, if you mean by castration, does he use ADT as a treatment regimen, of course, when it is evidence indicated to be the best for a specific situation. Thankfully we have many ADT drugs in the treatment arsenal, some that eliminate testosterone and some that block its use by the cancer that have proven safe and effective in many cases. You are very high risk, G9, and now likely systemic and perhaps advanced after failed surgery and SRT. The work of your urologist was over when the surgery failed. At that point, being G9 and young, it would have been good, in my opinion, to have brought in a medical oncologist like Sartor. But, now you know, and are getting on a proper, and, hopefully, an effective track.
