Make a list of questions, and let your doctors lead you forward!

Update on Pet scan in full
Placing me on 10 days of Bicalutamida followed by injection every 6 months of Eligard semestral when completed the pills, Review meeting with all the oncologists Thursday to plan the next course of treatment.

I was told that the 2 spots are extremely small.
here is the report. helps others follow my path.

November 24, 202l

Scan: PET-CT (64 C) WHOLE BODY

Radiopharmaceutical: 'l 8F-FLU0R0C0LINA (7.9 mCi)
Reason for examination: Patient diagnosed with adenocarcinoma of the prostate treated with external radiation therapy and high-rate brachytherapy.
Currently PSA figures of 3.94 ng / ml (previously of 0.857 ng / ml).
REPORT:

Technique:
Whole-body emission and transmission tomography was performed 60 minutes after intravenous injection of 7.9 mCi of
Fluorocholine. Transverse, coronal and sagittal PET tomographic images are obtained, without attenuation correction and corrected.
Description:
The images show the physiological uptake of Fluorocholine (liver, pancreas, salivary glands and to a lesser extent in spleen and
gastrointestinal tract) and its urinary elimination (kidney, urdters and bladder).

Cranial sections without assessable metabolic alterations in cerebral parenchyma.

Thyroid with normal metabolic appearance.
Laterocervical, supraclavicular, axillary, mediastinal, hilar and internal mammary chain lymph nodes without evidence of foci
hypermetabolic.

Pulmonary parenchyma without evidence of nodular images / condensations with pathological activity of Fluorocholine. No evidence of spillage
pleural or pericardial.

Uptake, n and intestinal elimination of the radiopharmaceutical with physiological characteristics.

Hypermetabolic lymphadenopathy is not seen in the mesenteric or retroperitoneal regions.
No pathological deposits of Fl8-fluorocholine in the prostate gland or in ganglionic territories of the pelvic region.

CONCLUSION

CONCLUSION

PET-CT study that shows bone metastases in the right vertebral body of D1 and in the spine of the right scapula.
Started with Bicalutamida 50 mg 10 days and Eligard Trimestral injection after pills. Review meeting for plan TBA
Size of both of them is extremely small.

Will know in few days when they have a review of situation with main hospital consultants what follow up treatments to do. No doubt will start after they have given time for the testosterone blocking to shrink it a bit.
I think they may do some target practice as they are very small. Will keep you all updated.

Update:
Injection after 3 months, then every 6 months of Eligard Trimestral to continue
Started on Calcium +D3 daily 1500mg, 400 UI (d3)
Apalutamida (Erleada 60mg) 4 pills a day to take at once, without food continuous treatment
11 Feb start of radiation treatment for scapula and D1 growths. .
PSA in last 3 weeks has dropped from 11 to 8. Blood test all normal readings.
No alcohol allowed from now on.

Bummer about the brew. Good drop in PSA. Sounds like a good plan. I expect a further drop in PSA following the Eligard and radiation. How many radiation fractions will you have for the scapula and D1 spots?

I am glad to see your psa is dropping.
Since currently we are located in the same country and it seems to are being given good care I am interested to know where are you treated, just for the case I need further help after my surgery four years ago. I have not so many references about good centers specialized in PCa here in Spain. Currently, I only have access to health care through private insurance (not access the public system, though I can switch if necessary).

I am based at Altea, 8 km North of Benidorm. I am a Spanish resident and being treated via the SIP National Health system. All radiology activity is sent to the Clinica benidorm as the centre of excellence at no charge to me especially if the treatments such as Bracytherapy is not in use at the main hospital at La marina Baixa They treat me via SIP and any drugs at €4.50 max per drug. My initial biopsy and consultation was at the La Marina and I am signed onto the oncology ward there from now on for any future decisions on treatment in cooperation with the Clinica benidorm oncology. Cant fault the treatment at all..

Thank you for your answer. I am glad to know you are getting the right treatment you need in a place so much close to your town. I wish you the best. Actually, I was considering to have HDBT in Alicante before I opted for surgery. As you may have guessed I am located in Madrid.

On 17th Feb I will have 2 gold fiducual markers inserted to the D1 vertebra to assist in the direct location of the tumor. Then a week later 5 or 6 directed radiation sessions at clavicle and vertebra locations. As an outpatient.

Started external beam radiation 8-10 sessions from today (weekends off) to attack the two tumor spots. Took about 20 mins. No of sessions may be less than 10.

Latest update, Blood test after completion of 10 radiation sessions and 8 weeks of erleada pills show PSA is now 0.2.
However bad Cholesterol has jumped up to over 300 and is either a false report or requiring attention. Previous levels have been within norms.My guess is the meal I had (Butter Chicken) night before blood test. So adjusted diet for following 4 weeks. Have small issue with mild headaches still that come and go. Can be eliminated with modest paracetamol dose. Had drops of blood in urine for 5 days during and after radiation treatment. Now ceased. Still not drinking alcohol. Could be the zero alcoholic beers that don't agree with me. Oncologist does not seemed alarmed and is delighted with results. Also continuing with calcium and Vitamin D. Have also just had the injection of Eligard Trimestral which will now be every 6 months. Erleada (apalutamida) continues daily 60mg. Overall energy levels ok and no issues.

It's good to hear from you - always thankful for your posts and updates.

I'm thankful you forged through your series of radiation treatments.

I can honestly say that my radiation treatments helped me a great deal, and continue to pay dividends.

I like the mantra that says, "It isn't always easy, but it IS always worth it!"

I feel like your update reflects that!

I give true thanks for that T-U-M-B-L-E in PSA values.

Just a couple of months ago, you were hovering around 10.0 in PSA values, and now you are down in those low decimal ranges.

That's affirmation that the steps you have taken are making a significant difference.

You have a multi-layered treatment plan in place. When I visit Dr. Kwon at Mayo Clinic, he is an advocate for a multi-layered treatment plan, when there are complexities involved.

(I always compare it to the snowball fights against my brother in the winter time when we were kids growing up --- it was good to have a variety of snowballs stashed away in my snow fort for my BARRAGE OF DEFENSE!)

It can take a while for the full benefits of radiation to be revealed. Your PSA may even continue to slowly T-U-M-B-L-E in the months ahead!

Thanks for reply. I keep this post updated because many may be terrified of Prostate Cancer and its blogs like this that have helped me track others and self inform on the journey and many have very good outcomes, probably the great majority. I get asked every day how I am. Kind of course but I am ok and get on with life. I see my cancer as a broken car, take it to a mechanic. I can't worry about what I cannot fix myself. In Spain I can only say that they have amazing National health care doctors who are right up there with the latest treatments and very quick at treatment. They freely co-ordinate with a private hospital with state of the art radiology equipment at no cost to me so I am in the best place.

Update 21May 22.
Review meeting today indicates PSA has again dropped by 0.01 to 0.2 from 0.21 in 1 month but cholesterol still too high. So starting on Atorvastatina (Statin) 20mg to see if can counteract what is believed to be a side effect of the Eleada pills. Had changed diet but made little difference to a cholesterol condition I have never had before. Readings are Colestrol 319 (50-200), HDL 42 (35-55), LDL 204 (80-130), Triglicerides 365 (50-200)
Such is life. The mild headaches have subsided at last and small amount of pain from radiation treatment on neck gone. Bring on the Salmon, and beans and Broccoli. !

Appreciative of your update. You are forging ahead!

I, too, was placed on a cholesterol-lowering medication a few years ago.

I eat a fairly balanced diet, and exercise daily, but it's a medication I need.

Keep living life to the fullest.

Your posts are GUIDEPOSTS to the importance of that!

Compaid, I am on Astorvastatin one pill a day for PAD. My cholesterol dropped down in my first checkup to an acceptable level and has stayed there. My best to you and watch your diet.

update 26 May 22
PSA has now reduced from 0.2 to 0.1 and cholesterol is showing signs of reducing with statins.Most showing 10% reduction.No problems.

Update
Latest blood test shows another modest drop in cholesterol to just above the max norms, started taking statin pills at night as common view is they work better if taken at night. Recently taken the trimestral 3 monthly injection.
PSA remains at <0.1 so 2 months now with no change.

Colestrol 308 (50-200) mg7dL, HDL 57 (35-55), LDL 206 (80-130), Trigliceridos 228 (50-200)

Have just undertaken another CT scan with enhancer just to check overall and found all clear, nothing located anywhere in body. Next review in 2 months.
Have also started a half dose of Lormetazepam 0.5 mg at night which has improved my sleep cycle almost back to normal. A problem I have had since being on the pills.
