Hi Sober. A bounce usually occurs 24-36 months after radiation, but if you have been on ADT for three years, like I was, any radiation bounce would probably have been offset by the ADT. You are more than four years from initial treatment so not when a bounce would usually occur, but you never know.
Your increase may indicate you are establishing an actual PSA given that you still have a prostate and one hell of a lot of testosterone. You may find it levels at some point and stays steady and that is good. You just have to live through several more tests to see what trend develops. I see your nadir, low point of PSA, was 0.10. That is good. Add 2.0 to that and that is the level where concern will move to action, usually in the way of scans to see if any cancer can be found.
What doc are you using now? No need to go back to the radiology oncologist as the only reason for more radiotherapy is if other tumors are found that can be radiated. You are nowhere near that at the present. If it were me , I would be seeking a really good medical oncologist that specializes in PCa. You were similar to me with 12/12 positive cores, high PSA and a lot of G4. That is pretty high risk. Get with a MO and be ready to pull the next trigger if you need to, but don’t allow yourself to be surprised.
