I have a 70 yo friend who was recently diagnosed with a 3+3=6 gleason in one poke / t2a. I suggested AS as the best possible short term way to go. He has scaring and repaired hernia from past colon cancer. I would guess surgery is not really an option. He is meeting soon with the radiologist to see about going that way.

What do you think? Is surgery out, is radiation the way to go. Is brachytherapy possible?

If it were me I would take the radiation at that age and with past surgery. But all options seem to be on the table, at least I think.

I think I would pass on surgery at that age and with his surgical record. Seeding could be the perfect solution for him if he meets the normal criteria including prostate size, etc.

I think robotic surgery will not be possible given the medical history and even open surgery may be so complicated that the better option would be Radiation of some sort either external or seeds.

Wouldn't hurt to ask about surgery. Additional radiation in the nether regions might not be fun.

Surgery may be problamatic for a 70 year old. We just don't heal as well or as fast as we did 20 years ago. Radiation is better, but still posssible complications.
The best bet is that he fits the Hopkins criteria for AS that would be the way to go.
