Some might find this useful: Dr. William Catalona's quest newsletter.

The current issue includes a defense of PSA screening, a discussion of why .1 is the cutoff for taking action on salvage therapy for PSA rise after RP, a new calculated index based on PSA and free PSA % for estimated the likelihood of high risk disease before biopsy, and a "risk count" based on PSA velocity. (as you may know, Catalona is a big advocate of the importance of pre-treatment PSA velocity, a topic that has been controversial within the PC research community.)

For those who wonder if I'm spamming: Dr. Catalona is not my MD and I have no commercial relationship with him. I'm just sharing this because I think there's some useful information here.

I can't get your link to work, it errors out, tried it 3x.

Would really like to read this, as my case is the direct result of the school of Pre-treatment PSA Velocity.

Finally got it to work, a most excellent issue.

On the PSA Velocity Issue, I am a believer, as both my Urologist and Oncologist feel the same way.

In the 12 months before my PC dx, my PSA rose from 4.5 to 12.3, then in just to months after, rose to 16.x.

In the articles about PSA Velocity, they consider a rise of .4 to be a dangerous signal, in that finally year for me, it rose by 7.8 points, really incredible, or, in the period of just 14 months, it had risen over 11.5 points.

In that line of thinking (PSA Velocity), it usually indicates an aggressive cancer, and that mortality from PC can be as much as 10 times higher, regardless of having a primary and secondary treatment. In my case, within a 2 year period from dx, I had both open RP and 72.0 gys of SRT, and failed them both. And now sitting with a fast rising PSA of 37.

According to my Urologist in particular, he said I was the poster boy for those that support the dangers of PSA Velocity.

According to my Oncologist, its one reason he's not pushing for HT at this time, as he feels that those that are in my situation will become HT refactory quickly, and he knows I have been dealing with serious side effects and Quality of Life issues as the results from all the failed treatments. He said he has seen cases just like mine that verify this thinking.

Just something to think about.

We so badly need an accurate diagnostic tool, that would show a specific variant from the start, though in my case, not sure it would helped any in making a treatment decision.
