poor quality seminal fluild after biopsy

Hi guys;

asking this for my hubby, on the shy side and not used to forums. He is on chronic Testoserone shots(always low) 2 years ago doc did PSA( he has just turned 55) Didn't like the 1st doc so went to a 2nd. Gave him ABT, in case it was Prostitis(sic). PSA came down from 4 to 3.7. They decided to do a biopsy. This was with a gun like device, he was awake, asked for a local(the doc said "if you want to be a big baby about it..." did give him something didn't let it take effect. Biopsy was neg, (thank you GOD) but it has been almost one year and his seminal fluid doesn't look ( almost like water) or in the same amount as before the surgery. I hate to ask b/c I know people will say "that is all is has to worry about" No he has other health issues, but the doc won't answer him if it was the result of having the biopsy. Anyone have a similar experience? Thank you so much!

I don't really think the problem (if it IS a problem) you're describing would be caused by or related to a prostate biopsy. It could be simply a function of getting older.

I also can't imagine why the doctor "won't" answer his question. I think I would call back and say, "look, you did the biopsy, now you need to reassure me that this change is not related to that procedure." I would be inclined to keep calling until you get an answer. However, he may want you to make an appointment so that he can SEE what's going on, rather than just guess at a cause over the phone.

Hi Nurse and welcome to HW. 

This does not sound normal to me based on my experience and what I have read for three years on this forum.  My fluid returned to "normal" within a few weeks.  I agree with Clock, i.e. you should demand an answer from your urologist.  If you are not satisfied with the answer then get a new doc.  You should not have to sit around and worry about this.

BTW, so happy for you that the biopsy was negative!

i, too agree, this does not sound normal or remotely related to the biopsy, not if you are talking about a year ago. i had 3 prostate biopsies spready over 18 months. the first 1-3 ejaculations after the biopsies, there was some dark distrubing blood in the ejaulcate, but that was normal and expected. there was some pain sitting down for a couple of days, but that was it. his doctor owes him a full and proper explanation of his situation if you all still consider it an ongoing problem. most men's ejaculate will return to "normal" in no more than a week or two at the most. good luck as you try to find an answer, and very glad to hear that the biopsy was normal.

as far as pain, it just varies too much guy to guy, as well as a person's personal pain level, and mental conception of what is taking place. my first one, hurt a bit, no pain med. second one, doctor put a couple of shots up there, and to be honest, the pain from the shots hurt way worse than the biopsy needles. so on the 3rd biopsy, i said no to the pain med, and just took it. for many of us, the loud snapping noise as the samples are being taken, are worse than the actual physical pain. however, there's been at least a couple of guys here, or perhaps more, that were put under or at least into a "twilight" zone, in order to understake their biopsy, everyone is different like that.

I know about getting older( my specialty is in geriatric nursing), however the quality of ejaculate changed right after the biopsy. This could be related to the fact his testosterone dose needs to be upped. I know he did say that the snapping was bad. Like I said felt a bit gulty asking about it b/c it was neg, but felt this was a safe place. i hang out on the MS, crohn and thryoid cancer sites on healing well. BTW my dad is doing well, dx with PC in 2000, went with external beam radiation and radiation seeding. Had a gleason score of 7 and 5 out of 7 nodes positive.

was just talking with the hubby, He said he felt the biopsy all the way down his penis, but the doc had a med student do it...Could this be part of it? My hubby heard things fromhis doc to the med student like " too far left" "too far right", " in too deep" Also he had bright red blood in the ejaculate for three weeks(no one told us.. they really didn't cover this in school so I didn't know). I wonder if it was b/c an inexperienced med student did the biopsy that he had and is having all these problems? Will have him call...or go in thanks again.

The blood is to be expected... Always helps if the UROs would point it out before hand.

I agree with Steve...the blood is to be expected and you absolutely should have been told.  I think most good doctors would provide that key info ahead of time.

Not to be paranoid, but with the med student angle do you really want to go back to the original doc for the answer?  Or, at least, go for another opinion if you are not completely satisfied with what he says.  As you undoubtedly know, some in the medical profession will not be entirely forthcoming about mistakes...

12 core biopsies in 2009 and 2011 (both negative) - seminal fluid returned to normal consistency within a couple of months. However, had 25 core biopsy in January 2012 (also negative) and since that day seminal fluid has been almost like water. Have also been curious as to reason for this, but did not ask uro during June visit.
