Hello: It is my understanding that once PCa has been diagnosed and you have surgery or radiation; free PSA no longer has any value as a guide. The total PSA signals that either remaining PCa cancer cells, benign tissues remaining near the nerves, or a radiated prostate are still producing it. The Free PSA number is often used with other factors and tests to make the INITIAL call as to whether a biopsy is needed at the beginning of our "journey". In your case (and as in my case), we just concentrate on the total PSA number and hope for the lowest possible. Good luck.
