My day just got better - my pathology report

I just received a phone call with my pathology report (surgery last Monday). I will get a copy later this week.

Gleason upgraded to 3+4 from a 3+3 (disappointed but not surprised)

Negative margins

Cancer did not extend beyond capusule

Lymph node  (left side by nodule) - clean (don't know if he took more than one)

Seminal vesicles- clean.

5% of prostate cancerous.

Other than the Gleason score I think everything else turned out pretty good.  MSK nomograms indicate a 96% progression free probability at 10 years.

Next on the list - PSA test in 6-10 weeks or so.

MrA being the same age as I when I had my procedure, I read your post with more than average interest. That your dissected prostate revealed more cancer than detected via biopsy is the exact reason I decided on a davinci prostatectomy. The what if factor weighed heavily.

I am pleased you have now passed on to our side of the stream, and by that I mean cancer free now and forever. I only wish at I could make that true for our brothers here. They suffer to a degree only my imagination can conjure.

Sounds very similar to my experience about 14 months ago.  You will find your recovery will speed right along and you will be walking in for that first PSA before you know it.  I've had four straight good ones, but I would be lying if I didn't say that the 4 or 5 days before each one aren't difficult. But worrying won't change a thing so enjoy being cancer free for now and concentrate on recovery.  Best wishes to you and your recovery.

I'm glad that your upgraded Gleason was not too dramatic. Probably nothing at all to worry about.

That does, at the same time, increase the growing number of patients in this forum reporting that their post-op Gleason report was upgraded from the biopsy report. Something for people to consider if they are pondering whether to choose AS as opposed to treatment.

Resident of Highland, Indiana just outside of Chicago, IL.
July 2011 local PSA lab reading 6.41 (from 4.1 in 2009). Mayo Clinic PSA Sept. 2011 was 5.7.
Local urologist DRE revealed significant BPH, but no lumps.
PCa Dx Aug. 2011 at age of 61.
Biopsy revealed adenocarcinoma in 3 of 20 cores (one 5%, two 20%). T2C.
Gleason score 3+3=6.
CT of abdomen, bone scan both negative.
DaVinci prostatectomy 11/1/11 at Mayo Clinic (Rochester, MN), nerve sparing, age 62.
My surgeon was Dr. Matthew Tollefson, who I highly recommend.
Final pathology shows tumor confined to prostate.
5 lymph nodes, seminal vesicles, extraprostatic soft tissue all negative.
1.0 x 0.6 x 0.6 cm mass involving right posterior inferior,
right posterior apex & left mid posterior prostate.
Right posterior apex margin involved by tumor over a 0.2 cm length, doctor says this is insignificant.
Prostate 98.3 grams, tumor 2 grams. Prostate size 5.0 x 4.7 x 4.5 cm.
Catheter out in 7 days. No incontinence, occasional minor dripping.
Post-op exams 2/13/12, 9/10/12, PSA <0.1.
Semi-firm erections now happening 14 months post-op & slowly getting a bit stronger.

Mr. Anxious, congratulations on coming through your surgery and on a good path report. To help you put your own situation in perspective, my husband's surgery was 5 years ago. His stage was T3a, Gleason 3+4 with one single focus of EPE with no other adverse findings. He just pulled a zero on his PSA test taken last week. In spite of the Gleason upgrade (which is quite common with surgery patients), I predict you will be just fine.

What a beautiful report!!!!!... Yes you can be anxious no more. Sounds like you are on your way be being well. Now time to start that walking. Congrats. News like your sure do brighten our day.
