Did the Gleason score you were assigned after your biopsy change after your surgery and the entire prostate gland was biopsied? Were you upgraded to a more aggressive Gleason score? Or downgraded to a less aggressive cancer?

I was one of the lucky guys that my g score at biopsy was 3+4=7. After surgery it came back 3+3=6. At this time 4 1/2 years out happy with my choice and outcome.

I went from a Gleason 7 (3+4) to a Gleason 9 (4+5). There was also substantially more cancer than expected.

went from 4+3 to 3+4 post surgery, yet cancer has acted very aggressive regardless

I sadly was upgraded from a 3+4 to a 4+3

Pathologist #1 read my dad´s slides as Gleason 6, but another expert pathologist (only prostate) read it as Gleason 7 and 8. A few years ago, Gleason grading was updated, so Gleason 3 became Gleason 4, Gleason 2 became Gleason 3 to more accurately describe the actual disease behavior, Dr.Epstein described this change in one of his papers. In my father´s report a "cribiform pattern" was mentioned. Dr.Epstein wrote that finding this pattern anywhere in a sample automatically meant it was Gleason 4. I have to wonder if pathologist #1 was simply not aware of these updates to Gleason grading.

My biopsy indicated a very small amount of 3+4=7 but my post-op pathology downgraded that to a G-6. However, I question that since I appear to have a recurrence underway and I thought that was virtually impossible with a G-6.

Gleason 6 can recur. It is distant metastases that are virtually impossible.
You had positive margins which means that some of you G6 cancer might have remained in the prostate bed (the surrounding tissues) after your surgery. It can sometimes go on growing there -- thus a recurrence -- but as long as it remains G6 it is not likely to move to your bones or liver or whatnot. If your recurrence gets to the point where your doctor wants to pull the trigger it should be fairly easy to nuke it with radiation since you should have a good idea where it is.

Similar to Peter ..
I was a 3+4 heading into surgery .. came out the other side as a 5+4 .. yikes!

There has been study after study after study already done on this. Google it or check out pub med. The chance of upgrade after surgery is 30%-40%. Not sure what the point of this thread is.

Yes, the stats are available for the differences between post-op pathology and biopsy for radical prostatectomy. The active membership of the HealingWell forum is too small to replicate those studies and is oddly self-selected. This sort of thread is interesting the other way around. Since we know the percentages in the population at large then polling the membership of the forum can tell us things about the type of person for whom participation here is rewarding. In the similar thread on Gleason scores we had more than twice as many G7s as G6s, and just as many G9s as G6s. Compared to the world-wide population of men diagnosed with PCa we are topheavy here at HealingWell. We have fewer G6s than you would expect and way more G9s. So that other thread suggests that men with higher Gleason scores find participation here more compelling and that is why they are proportionaltely over-represented.
This particular thread seems to be closer to the expected result which means that one's Gleason score going up or down or sideways in the post-op pathology doesn't affect a man's inclination to be active in this forum one way or the other.

I've found on Internet forums dealing with medical issues that the results of polls can be very skewed i.e. unresolved medical problems hence seeking support/answers. The people that have healed/been cured/etc. tend to move on with their lives. It is the nature of the beast.
