The perineural invasion (PNI) thing is fairly common and not necessarily a super worrisome finding, but still nobody wishes to have that finding. It is never dumb to get more opinions on this, realize the rules in PCa- they say there are no rules, there are generalities and even exceptions and anomalies. Check Partin tables and nomogram statistical data to compare what your odds are for curative treatment, so you atleast have an idea and use your game plan accordingly, lots of ways to fight this dragon.
