Imaging options

Hello Everyone
This is only my 2nd post. My dad was just diagnosed with PCa via biopsy. His PSA is 27, Biopsy result: Right lobe 10/10 benign Left lobe 10/10 Gleason 7(4+3) 90% of each core. He has no symptoms and it was uncovered via a routine blood test. We have an oncologist appt tomorrow.
At this point, I assume some additional scans/tests would be required to determine whether its organ confined or spread externally, before a treatment plan is laid out.
Can someone please help me out with what imaging techniques are in use today to determine metastasis?
Is there a specific type of MRI or PET or other scan we should be ensuring he gets?

Yup, they'll be likely ordering some scans. MRI of the prostate bed and a bone scan (PET) to look for mets are likely. There are other blood tests, free PSA for one, but the elephant in the room is a PSA of 30. I'd expect a lot of the conversation to be about that face. Hard to say if any scan will turn up anything but at least you'll need a base line.

The bigger question is treatment selection. HT is likely to be recommended at his PSA level - whether or not it's organ confined.

and it's not common to get many symptoms with PC before treatment. Until the bone mets are advanced or a tumor blocks the bladder or something, there isn't a lot of symptoms felt. Most PC is discovered through routine screening.

Hang in there. Even though it looks scary, there is plenty the doctors can do for your father and he'll be here for a long time. He may not like the treatments but there are treatments that can be done.

I'm researching all these scans you mentioned.
In case it has spread does surgery have any negative effect in terms of the ability to control the disease?

I ask because I read somewhere that the primary tumor releases something called angiostatin that prevents smaller metastasis from growing blood supply vessels. However when the primary tumor is removed, the small micrometastases now have a chance to grow in the absence of angiostatin. I don't know if this applies to PCa. Although we do wish for the tumor to be removed from the body, I wish there was some clearer guidance on whether surgery should be considered, not an option in certain cases.
