It's just not a very good test, although it seems to be better than PSA alone.
Here's a review of all the research done on it to show whether it can be used for initial biopsy, repeat biopsy, or to indicate virulence. Some have suggested that PCA3
density may be a better indicator, but that remains to be validated. Its best use is probably as PCA3 density along with other biomarkers like PSA density, PHI, and other biomarkers that insurance probably won't cover (e.g., 4Kscore, TMPRSS2:ERG fusion, other genetic tests).
