Greetings

In the process of testing for SRT to begin for upward moving PSA. Current reading is .06 at 88 months out from surgery. 3+4 T3a Scans all clear for cancer however radiologist found signs of avascular necrosis in pelvic area and said for me to find a orthopedist to evaluate. Radiologist said it may alter treatment plan. Anyone here have had this situation? What was the outcome?

I think you are making a mistake getting RT, but that just an opinion I am entitled to make. Im sure you know my reason. what ever you do I hope it works for ya. I have no answer to your main question.

I would be investigating some advance scanning before doing SRT this far out from surgery.

Is the psa progression (doubling time) rapid?

My ART did more damage (that I can see 4 years later) than surgery, so I would want to be seriously convinced.

Is the current plan to radiate prostate bed and pelvic area?

I would wait for the time being. In all practicality you are still <0.1 meaning undetectable, and at 88 months out your doubling time is very long. My oncologist urologist told me not to do anything until I hit 0.2 and then a following confirmation test before I think about any further procedures.

Remember the longer you can wait the better the science and procedure to improve efficacy with reduced secondary induced problems.

Not understanding your logic at this juncture? what am I missing?

You are almost 7 1/2 years out, and your PSA is only .06? Did you mean .6, instead of .06?

If it's truly .06, why do you and/or your doctor feel that you would remotely need SRT? You haven't even reached the bare minimal threshold of detectability of .10, in fact, you are only 60% of the way after 7 1/2 years.

So why would you want to subject your body to rigors and risks of radiation when you are clearly still non-detectable?

Perhaps there is more to the story.

I concur with what the others have said. I had my SRT done after my PSA went from .064 1 month post. RALP, .074 3 months then at 9 months it jumped to .33 at that time only after my PSA drastically made the jump from basically a zero to .33 test they did something...1 month post SRT my PSA was down to .04. So if you are still at a.06 IMHO you are still good?

.06 is too early to think of SRT after many yrs of undetectable PSA. Radiologist will advise after 3 tests of elevating 0.2 for opt to SRT.
My case, elevating psa told me is time to seek treatment. I m a G9.

Question --- don't I remember that TA posted on a study that showed that not waiting for .2 , and considering SRT at PSA .03 has been shown to have a better result , I.e. Better chance of cure?

Maybe I'm mistaken I thought that was the subject of a recent thread.m
Or maybe the fact that it took over 7 years for the rise to occur makes Octorob's rise, less of an issue at his level?

You are correct- there was an article about early success for SRT starting at .03 instead of waiting until .2. I am going to orthopedist today to discuss "bilateral avascular necrosis in femur heads" which was found on MRI. My RO sending me. This was my original question for this thread: has anyone else had to deal with avascular necrosis before SRT?
