That's a tough one. Never good to see an upgrade of one's Gleason. There is a world of difference between 3+4 and 4+3.
The real question is, was all the PC confined to the prostate. If yes, you're done. If no, you'll need more treatment. How to know? Watch the PSA numbers.
Should you treat before the PSA starts rising? Don't know. SRT would be the normal next step if you needed more, but I'd be tempted to wait for one more three month PSA test before deciding.
Also, are you getting th ultra-sensitive PSA tests? They should be able to see lower than .05, which might be meaningful in your case as the newer studies are suggesting SRT with a rise over .03.
