Hi Islander-
You encountered what we've all had to deal with - there is no single right answer and it's up to the patient to make up his mind. Johns Hopkins is justifiably known for surgery and certainly has a bias towards it. That doesn't mean surgery isn't a good option - it probably is - but it's one of many. The make-or-break deciding factor about surgery, imho, is whether the cancer has penetrated the capsule (Stage T3). If it has, I think surgery is a terrible idea.
We share the same urologist - Leonard Marks. I think he's excellent, if a bit brusque. He probably steered you to SBRT (with Dr. King) or HDR Brachy (with Dr. Demanes) - two of the tops in those specialties. I strongly recommend you talk to them if you can be treated in LA. Both of those have been very successfully used as monotherapies. For seeds, I think most specialists would recommend external beam with it because of your "unfavorable intermediate risk."There are many excellent surgeons at the top tertiary care facilities in the LA area - Cedars, USC, UCLA, UC Irvine & City of Hope.
