The NCCN Guidelines are slow to change, but the inclusion of many 3+4 men (their
other
case characteristics also need to be considered) in AS programs has been going on for a number of years. There's also been recent expansion, in some programs, of men with PSAs up to 20 ng/mL...again, based also on
other
case characteristics.
Perhaps more than anything, the NCCN's 3+4 change helps clearly show that men who opted for surgery and were upgraded to 3+4 really shouldn't worry at all...in most cases they would have still been eligible for AS!!!
