Are you with a world class urologic oncologist that has demonstrated experience and success with high risk advanced cases? If in doubt, get to one.
I would have hoped for a greater drop in PSA than you have experienced. While I was a G7, I dropped from PSA 59 to undetectable in a little over 90 days on Lupron and Avodart. Also, he should have his T checked to determine if he has reached castrate levels.
Then there is the issue of whether another treatment should be introduced like chemotherapy. Also, is he screened for Provenge? I would have everything in order to go on at the very earliest opportunity.
G10 is as aggressive as it gets, the treatment should be more-so, in my opinion. Another member, JohnT, has often said that "low risk PCA is over-treated and high risk PCA is under-treated".
Best wishes,
Jack
