You're raising a very good point. 95% of prostate cancer mets may express PSMA to varying degrees, while 5% do not. The first 3 on the list are PSMA scans. Their high detection rate may be attributable to the fact that many early metastases are in lymph nodes. The NaF18 PET is specific to bone, and is useless for soft tissue mets. It should get picked up by all "blastic" bone mets, and most PC bone mets are blastic.
The one on the bottom of the list, FDG, may actually be the best one for detecting the most advanced metastases. I put it on the bottom because most guys with mets want to get their first PET scan at an earlier stage, and by the time that an FDG PET can detect it, a bone scan/CT can probably detect it too.
The non-PSMA PET scans now being tested may be more sensitive and specific.
