I’d like to start by thanking everyone who has posted here over the last several years. The information, along with some of my own research was enough to convince me the the Gat Goren procedure was the best option for my particular situation.

Now to return the favor, I’d like to make everyone aware of a physician in Arizona that can perform the procedure. He did mine and the entire experience was great.

I was treated by Dr Joel Rainwater at Comprehensive Integrated Care in Gilbert Arizona-just outside Phoenix. CIC does not advertise the procedure but during my extensive research I found an old newspaper article about Dr Rainwater being trained on the procedure by Dr Gat so I contacted him. He is very nice and an excellent communicator and we had a great phone call. After we talked, I forwarded my recent 3T MRI of my prostate, PSA and other relevant labs. On review he felt I was a good candidate and we scheduled the procedure shortly afterwards after we both reviewed our schedules for an opening.

I flew in the night before, stayed in a hotel in downtown Phoenix then arrived early in the morning the next day (they are about a 25 minute drive from downtown). You will need a driver or other way to get there because they use light sedation to make the time on the table more comfortable. I was back at my hotel about 3 hrs later and felt well enough to go for a walk shortly afterwards and then join a friend for a late lunch at a local restaurant. I had no pain at all after the procedure.

A previous Doppler U/S done to look for clinical varicocele was negative but my understanding is that this is not a contraindication. The theory is that sub clinical backflow of blood through leaky valves can lead to BPH while not being great enough to visualize on U/S.

Prior to the procedure I was waking up 2-5 times a night to void, had obnoxious post void dribbling, slow stream and a sensation of incomplete emptying. My prostate volume was estimated at 59 on MRI which is about twice normal size. I’ve never had a PSA>2.5 and was not on meds prior to the procedure. I had previously tried alpha blockers and dutasteride without much benefit.

To my surprise I noted some benefit at one week post procedure even though I did not expect any for several months. I’m now almost 2 months out and my rate of flow and dribbling are substantially better though not yet totally normal. I usually get up a couple of times at night to urinate.

The cash pay cost was just under $8,000 and I consider this money well spent. I’ll try to update on my progress periodically. I anticipate much more improvement over the next 12-18 months but we’ll have to see if that pans out or not.

You should first treat the gut dysbiosis that is probably causing your varicoceles. I did not find out just how dysbiotic my gut was until after I had the Gat Goran operation. There is a co-lateral route, that most Drs do not know about, from the portal venous system, around the gut, into the Gonadal (testicular) Veins via the Superior Rectal Vein and Veins of Retzius (Dr Andrew K Burroughs) paper on portal hypertension) at the upper rectum...where I had terrible pain while passing stool prior to having the Gat Goran procedure. Gut dysbiosis is hard to beat. It takes a change of diet away from carbos and fruits to reduce the sugar up take....or even strong antibiotics, faecal transplants or phage therapy. I have done them all and years later I am still trying to win the battle against the bad bacteria in my gut. Phage Therapy has been amazing for me.
