My understanding is that Xtandi is a second-line hormonal treatment, usually given once the first line hormonal treatment (Lupron) stops being effective. Many times, a choice is made to go with either Xtandi or Zytiga, and sometimes the decision actually gets made by what the insurance company is willing to pay for. One difference between the two is that with Zytiga, patients usually also take Prednisone. For me, and many other guys, the Prednisone tends to stimulate appetites among other things. Side effects are part of the game and finding the right balance with effective treatment is a challenge. You may want to talk to your oncologist about maybe trying Zytiga.
