Thanks for the report. What was your post-op pathology? That will give you some idea of the future, although the best information will come from your PSA tests.

I'm happy you're comfortable with your decision on surgery. It isn't an easy choice.

Take your time and get back in shape. The worst may be over!

Glad it all went well, Audio. I, too, am curious as to what your final pathology read (although I see your LNs were clear).

And I'm sure you'll be fine. After all, my daughter was born at GW (the former building across 23rd street) after an emergency C-section over 20 years ago and is doing great! smilewinkgrin

thanks for the responses. i don't have the path report here at work. Will get numbers later.

With that said, Engel basically said that with clean lymph nodes and the numbers in the report, he would put the % chance of me having a PSA greater than zero in 3 months at less than about 5%. Also, he and i have a pretty good dialogue and when we went over the results and I began asking all of the questions I had read about to ask (e.g. margins, etc.) he basically said at this point, all of that info means nothing. Once we get the PSA numbers back at zero is the time we can feel comfortable and, once we are comfortable, the other data in the path report will be irrelevant. So no use is stressing over them now.

Same here and I haven't regretted my decision in spite of my positive node. I thought it was valuable to have the prostate out for pathology and indeed the result was a trigger for more treatment. Also the Decipher test indicated I was higher risk.

OK, here is some info from my pathology.

Lymph nodes R and L - negative for carcinoma

Gleason score of 7 (4+3) with a tertiary pattern of 5, involving 20%
- Gleason score is based on the dominant nodule of cancer present in the right prosterior aspect in 8 consecutive slices. Additional foci of cancer with gleason patterns of 3 and 4 are noted elsewhere;

Prostate - 20 grams; 3.7 x 3.6 x 3.2 cm

Tumor: % of prostate involved by tumor: 20% (present in 12 of 30 slides); tumor size (dominant nodule in the right posterior aspect in 8 consecutive slices, 3mm in thickness each) - 24mm;

Urinary bladder neck invasion - not identified;
Seminal vehicle invasion - not identified;

EPE - present, established - right prosterior, toewards apex

Margins: involved in invasive carcinoma (not at site of EPE); Limited (<3mm); Focaillty: multi focal; Gleason pattern at Positive Margin - 3

So, this is the info that Engel reviewed and said glad we go this out, surgery was defiantgley the correct source, your physical probably saved your life and lets wait for the zero PSA in 3 months.

Here is another audio guy in Houston. Your post op path report really does verify that your physical really has impacted the opportunity to save your life. A higher Gleason of 4+3, tertiary 5, EPE, and positive margins should validate the effort you put in to seek information, weigh the options and make a choice accordingly. I doubt that any knowledgable person here would not agree that these numbers are life threatening if not addressed. Let's all hope and pray for a big < (less than) psa number

Thanks. I know i'm fighting. We'll see what comes and deal with it. Working with JHU and GWU so got smart folks noodling it.
