IMRT 7 years post surgery?

Went to my surgeon today. Surprising visit. After 7 years of 0.1, my PSA went to 0.2 six months ago, then to 0.3 two weeks ago. My expectation was that he would refer me to a radiation oncologist. Instead, he said that with the 7 years of low, stable PSA results, my potential recurrence was probably "distant", not in the prostate bed and, thus, radiation would not be effective.

His recommendation was to do one more blood test in three months and if it goes up again, consider hormone therapy. Anyone have a similar experience.

It's possible he's right.

You might want to get an opinion from a medical oncologist who specializes in PC. Not sure what an RO would say in this cause, but a second opinion is always in order.

His input makes complete sense.

BTW, you've had a great run, and it will continue. Five years post-surgery with primary Gleason grade 4, tertiary Gleason grade 5, and EPE. That's really good! And only now early signs of a possibly very slowly increasing PSA. Very good! HT may very likely be in your future...maybe next year, maybe the year after.

Your PSA may be high enough to be detectable on some of the newer PET scans. You can get the FDA-approved Axumin (fluciclovine/FACBC) PET scan at UF Jacksonville (contact details in link below). It should be covered by Medicare, but check first. If no distant mets are discovered, you may still be a good candidate for salvage radiation.

You should definitely see a radiation oncologist on your own to discuss this. Your urologist (who should have referred you when your PSA was 0.1) has no way of knowing if your spread is distant or if it is still localized and curable.
