1 Year Post Surgery PSA

I got a call from my doctor today (1 year anniversary of robotic prostatectomy) that my PSA is .03. This is trending upward from .02 at 6 months and <.02 at 3 months. Doctor will test again in 3 months. If it continues upward, he mentioned radiation as a treatment. What PSA number should I start getting nervous about?


Surgery at 50 now 51
4/2017 - 1 year PSA is .03; @ 6 mos was .02; @3 mos was <.02 (undetectable)
4/2016 - DiVinci RP nerve sparing
2/2016 - 8 of 12 cores positive (T1c)
1/2016 - PSA 11.1
Continence is Superb
ED is still a problem - I inject BiMix

Injector - welcome to the forum. What was your Gleason, did you have positive margins, was the tumor contained, what was your stage, seminal vesicles and lymph nodes clear, doubling time of PSA? All those things affects the decision when to start SRT. Many doctors would wait until PSA is in the range of 0.1-0.2
Some would start even earlier depending on the listed aspects.

Welcome to HW.

There's no specific number, but watch the trend. If the retest in three months is higher, you need to get to an RO for SRT.

Sounds like your doc has you on the right path here.

Guys, thanks for the comments. More details you asked for:
I had stage T1c, 8 of 12 cores positive. Gleason was 6 on 7 cores and Gleason 7 on 1 of the cores. Margins were negative, lymph nodes and seminal vessels were clear. PSA took 5 years to get from 5 to 11.1. Lab work has all been done at same facility.

The next PSA test will tell. Hopefully it won't go up. But there are worse things than a bit of radiation.

Since you had favorable pathology (no positive margins and stage T2), the thing you should look for is a pattern of steady PSA increases or an increase in PSA of over 0.05 ng/ml/year.
