Psa doubled in 1 year

BPH, or prostatitis could explain the rapid rise and PSA level. Actually, prostatitis could account for many of your complaints and is a real possibility. When your doctor did the biopsy they should have measured the size of your prostate and that would tell you if you have BPH.

At a PSA of 4.9, most PC would not have any symptoms. PC only has noticeable symptoms in the late stages, and at much high PSA numbers than yours. The nodule could just be calcification.

Sorry, I know you want answers, but there isn't enough information here to say anything for sure. Anything we offer is just speculation and that's not really helpful.

and I've had hemorrhoids - that alone causes plenty of discomfort. Hope you're getting some treatment for that.
