If your biopsy has tumors in the apex, does that make surgery and getting "clean" margins more difficult.

How much weight should that be given in making a treatment decision. Just curious, as I've already been microwaved

I had a tumor in the Apex.
As it turns out, I had a positive intra- margin on intra-operative frozen section, leading the surgeon to cut some more until he got positive FINAL margins.

My understanding is that the apex is the most common sight of positive margins, because there is no capsule, or there is less definition of where the prostate ends, at the apex.

about 50% of all positive margins occur in the Apex, so the risk of a positive margin is much higher than a transition zone tumor or a normal PZ tumor.
It was a major factor in my decision to have radiation instead of surgery.

From what I was told if you have positive margins, it is best to have them at the apex then the base which is next to the bladder.
