According to the Ahmed/Emberton group in the UK that has the most experience with FLA, they
only seek out intermediate risk guys, like yourself. They believe low risk guys guys should prefer AS, and that it is a questionable therapy for high risk guys because of the high do-over rate. Your having only 2 positive cores would seem to qualify you as well, if they were in the same lobe of the prostate. The mpMRI will locate the index lesion and will tell them if there are any other significant foci of cancer. If the index lesion is close to the urethra, the rectum, or the bladder neck, you may be excluded.
Whether insurance will cover it is another question. If it's part of a clinical trial, as it seems to be, they may sometimes cover it - but it would be the exception rather than the rule. It's usually about
$20,000 and they give you a discount on the do-overs.
Whether it's a good idea or not depends on you and whether you are willing to take a risk on a new technology with limited follow-up. Please read the following carefully:

In Chicago, I suggest you set up an appointment with Arica Hirsch at Advocate Lutheran. She specializes in a kind of radiation called SBRT that is completed in only 5 treatments. Hormone therapy is unnecessary for intermediate risk guys, the cure rates are very high, and the side effects are very low (particularly low rate of erectile dysfunction).
