I had a multiparametric 3T MRI for staging. From what I learned at the time, the limitation of MRI is sensitivity. It may miss small things that are there. However, if it sees something, it is likely to be there.
My report said: Abnormal signal and contrast enhancement in the base and mid
segments of the peripheral zone of the prostate, predominantly on the
left in the 2:00 to 6:00 axes, with an appearance that is suggestive of
left posterior extracapsular extension with possible neurovascular
bundle involvement by adenocarcinoma.
My doctor's comments on that were supportive, in that the "suggestive" appearance on MRI is rather reliable indicator that it's actually there. Generally, the MRI is more likely to miss things than to see things that are not actually there. Combined with several biopsy cores that were 70% to 100% involved, that all seemed consistent.
