Prolaris is an expensive test. Get preauthorization from your insurance before you get it. It is only used by men who are "on the fence" over whether to be on active surveillance or not. If you are not considering active surveillance, there is no point to the test.
With your "favorable intermediate risk" prostate cancer, you have many good treatment options that all have about the same excellent success rate. There is no rush to decide. Take your time and investigate each option thoroughly. Sometimes access to excellent specialists precludes a therapy. You should investigate surgery, low dose rate brachytherapy (seeds), high dose rate brachytherapy (temporary implants), and SBRT (IMRT only if SBRT is unavailable). Because you only had two positive cores, both on the same side, you may be a candidate for focal or hemi-gland ablation too.
With your type of prostate cancer, the probability of recurrence is less than 10%. All therapies have salvage therapies. After surgery, you would do salvage radiation. After radiation, you would do salvage focal brachytherapy or SBRT or ablation. After focal ablation, you could do more ablation or salvage radiation.
