Back in August, I began the process of deciding whether to participate in a clinical trial:

My medical oncologist is offering me an opportunity to participate in a clinical trial with three arms: (1) degarelix (Firmagon); (2) degarelix + apalutamide; (3) degarelix + apalutamide + abiraterone acetate (Zytiga) and prednisone. Assignment to one of the three arms is random so I have a one-third chance of winding up in the control arm that gets degarelix only, a two-thirds chance of getting apalutamide in addition to degarelix and a one third chance of getting all three drugs (plus prednisone to mitigate some of the side effects of abitraterone). In each case, the drugs would be continued for one year and then I would go on a vacation.
I subsequently considered the alternative of metastasis-directed radiation therapy but a Ga68 scan turned up no metastases at which to direct the therapy, so I'm ready to take the leap into the clinical trial. The NP at UCSF told me that they used to recommend a bone density test before beginning ADT, but don't do that now because Medicare no longer pays. She said it would cost me $700-900 if I wanted to do it. This seemed way out of line as it appears that Medicare reimburses in the neighborhood of $100 for a DXA scan (and fitness DXA scans using the same equipment can be had for $45). I have since found a nearby provider (Northern California Institute for Bone Health) that charges $395 for a DXA scan (if Medicare won't cover) and an additional $150 for a Trabecular Bone Score which is apparently a report that reanalyzes the DXA data to analyse the quality of the spinal bone).
My questions are:
1. Does getting a baseline bone density test before ADT have any clinical value?
2. Does the study recently published in JAMA that suggests calcium supplements are not effective in reducing bone fractures affect the answer to question 1?
3. Is there anything else I should do before beginning a one-year course of Degarelix (with possibly as many as three other drugs on top of that)?
I have many fears about
ADT but I recently took a couple of months off from worrying about
it and my plan, once I take the leap, is to simply to do everything I can to maintain good exercise, diet, and sleep habits. Thanks for any advice!
