I am 52 with PSA LAST CHECK OF 3.4.
I had couple of weeks back got my biopsy result from local urologist that showed 10 places with cancer, 2 with 3+4 and the others 3+3 with recommendation of surgery.

today I had my second opinion meeting today with top medical oncologist at Dana Farber today and the reading from their pathologist (2nd opinion) was all 3+3 ( the 2 3+4) were not correct. I was so happy for a few minutes but then was told that because of the 10 places 3+3 volume, that surgery is still definitely recommended and this is not a place for Radiation or active watching.

so I am meeting with surgeon in couple of weeks - just wondering if anyone has thoughts in agreement or not on this point. I know it is not that clear based on some of the readings but the top oncologist had no doubt that surgery required. do not want to take chances

I agree that with 10 cores (I assume 10 of 12), Active Surviellance is not a good idea. But I'd get another opinion on radiation, from a Radiation Oncologist.

Surgery isn't a horrible choice, but it's not your only one. RT is an option. Mystifies me that a oncologist would say that. See an RO before you decide, you'll get a different opinion.

Still, if you want surgery, that would work.

You should not be talking to a surgeon or a medical oncologist about any alternative other than surgery or active surveillance. I have no idea why someone with curable PC would be talking to an MO anyway - they have no expertise in those therapies.

Get your slides read by Epstein for a definitive answer.

There is nothing about your biopsy results that should preclude AS. You have to understand that every hospital has its own rules about AS. NCCN recommends AS as an option for anyone who is low risk (as you now seem to be). There is no exclusion for cancer volume, at NCCN. DF is an NCCN hospital, as is Johns Hopkins, but elects to follow more stringent criteria, apparently.

You have plenty of great choices in Boston, and you owe it to yourself to go out and talk to a few. i recommend starting with Irving Kaplan at Beth Israel.

Tomo why are you starting a new thread when you started the same exact thread back on 3-21? You had lots of guys give you support and lots of advice from personal experiences?

skypilot my situation changed today based on second reading of my biopsy scans - that is why ok?
