I’m still trying to understand my last psa numbers, please help

I have completed 26 sessions of imrt + HDR brachytherapy(1fraction)+ on going Lupron injections(18 months)my initial psa was 26.6 , first 3 months down 1.1 , 6 month 0.4 which was about
2 weeks after my HDR brachytherapy. My question is should I be undetectable at this point or is my numbers ok at this point in my treatment?

You're doing fine. with RT it can months/years. As you get further on HT, it likely will go down further.
Right, you just need to monitor.

Your PSA will likely
never
become undetectable. Undetectable PSA is something that guys who have surgery, not radiation, can get. Since you have an intact prostate and prostates put out prostate specific antigen (PSA) you will always have PSA.
I'm frankly surprised that your PSA was that low only 2 weeks after brachytherapy. It irritates the prostate and causes more PSA to be output. However, any nadir below 0.5 is prognostic for a good long-term outcome.

I thought that since I was on Lupron it should be undetectable.

Glad to see your treatment choice in your case! the PSAs that matter will be after testosterone returns. good luck! you can see my PSAs below. I was not PSA tested while on Lupron.

The lupron is working. You're doing great. Your PSA will likely rise as you come off the lupron.

The Lupron is what kept your 2-week post-BT PSA so low. PSA is caused by prostate irritation in addition to PC, and your Lupron doesn't change the other sources of PSA.

As several others have pointed out, your PSA test result looks just fine. No reason to worry or change the protocol.

No - what good would that do? You should have your testosterone level checked periodically.
