That is called "salvage surgery," and it is seldom a good idea. Although if you're going to have salvage surgery Eastham is about
as good as it gets. If you should have a local recurrence after radiation, much beter options are salvage brachytherapy (which Zelefsky can provide) or salvage ablation (which Jonathan Coleman, also at MSK can provide). If you want to read about

Also about the degree of difficulty with reoccurrence and revision surgery The dose to surrounding organs is figured out after they get a good picture of your anatomy using a fused CT/MRI after fiducials have been inserted (for external beam). For brachytherapy, they capture the image intra-operatively. They set "dose constraints" so that organs at risk do not receive high doses, and they come up with a "dose volume histogram" that shows the dose they will get.
