I had my slides sent to Epstein after RALP on Nov 7.
His findings were all similar with the exception of the diagnosis of Intraductal Carcinoma of the Prostate.
Can’t find a whole lot about
it but it doesn’t sound good. There are a few threads here. I’ve read a dozen or so published articles that talk about
pathologists making the distinction and that it tracks with poor outcomes.
Any thoughts from those of you that have been around? Anyone else here have this same diagnosis? What was your treatment after RALP? Seems like the papers I read indicate hormone therapy does not help or work for this type of PC.

The good thing at least is that you have some downgrades - esp the one from 4+4.
I have seen a few threads on Intraductal on HW in the past but with no different treatment path. It does carry higher risk.
I would take second opinions from a research based institution as this is not very common.
Don’t base your decision solely on what you hear from your Uro or what you read up as you are in somewhat of unknown territory to most.

As a follow-up I sent an email to Epstein at Johns Hopkins about
the Intraductal designation on his report.
He responded within an hour which I find amazing. He said the designation was for the tumor contained within my prostate and that I need not be concerned about
it with regard to treatment decisions.
My surgeon follow-up and PSA result is today. Hears hoping for the magical undetectable result!

I have IDC-P as well and was also diagnosed by Dr. Epstein. If you search on IDC you'll see all my posts.
I was at .05 but now am at .33 with a psad of 1.44 months so I'm going back on active treatment this month (most likely lupron again). I was off lupron for six months and psa stared rising.
All I can say is probably everything you've read..rare, aggressive, no standard of care, etc.. Any doc will simply treat it with the same protocol as high risk. I had chemo then ebrt then HDR brachy all the while being on lupron.
