I had my annual physical (61 years old) and by PSA increased to 20. I have had three biopsies (2006, 2009 and 2013) and all came out negative. Additionally in 2016 I had a "FirstScan" done (full MRI) which also showed no signs of cancer, but an enlarged prostate - which it has been since 2006. My PCP wants me to go back to the urologist, but I just don't know what else they can do. How concerned should I be? Thanks for your input.

What were your prior PSA"s?

I would request the prostate health index, PHI, blood test which is a pretty good predictor of prostate cancer.

20 is high, but not extraordinary for BPH. Have you ever taken anything like finasteride (flomax?) Do you have any BPH symptoms?

Urologist - Yes! Do it. Its been 6 years since your last biopsy. Also Bobby Mac gives good advice about the Prostate Health index.

PSAs have been low to mid teens for last five years.

When first discovered back in 2006 I tried Flomax for a while and also Avodart for about six months each with no impact.

I have not heard of the BHI before. So I will look into.

It is just very frustrating!

I would get another MRI. I went 10 years going through what you are. Transition zone tumors throw off high psa. They are difficult to find with a biopsy.

Welcome to HW. The PHI is a better test.

You should find out what's up, but an enlarged prostate is adding to the PSA.

You might try a different URO this time.

It is frustrating not to get the answers you want.

I agree with Andrew - see another urologist this time. (personal experience)
