I was given the 4KScore test last Dec by my Uro. I just got a EOB from Medicare saying they don't cover that and the cost is $760. If my Uro didn't inform me in writing about the non-coverage they say I don't have to pay. He didn't.

Anyone else get this covered by private ins.?

Could be that you are over 70? The brief reading I did seemed to indicate that there might be a criteria for the test. You might try contacting the test company as many have reductions for those whose insurance does not cover cost.
