AS-spreeading out BX

Hi all
On AS and this June Had my 3rd biopsy
Summary
DRE 3-18=nodule, 3-19 & 20-slight induration
PSA- between 3 and 4.3 no trend but on 5-20 kicked upt to 4.3 from prev 3.2, follow up 1 mo later 3.9
BX 4-18=1/12G6 5%,rt apex, 6-19=15neg, 6-20=1/15.G6<5%, Lft apex,2 cores showed inflamation
MRI 6/18 & 1/20=pirads 2, no focal lesion

Have issue with low flow taking flow max, had urodynamics, renal ultrasound and 2 uroflowmetry-some concern emptying bladder and want to monitor
was on antibiotics for inflamation(prostatitus) for 3 wk

Bottom line given the lack of any new findings does it make sense to spread out biopsy but do MRI, will using 4K test add any insight or be useful for monitoring. Could biopsy aggravate the inflamation or could it have even caused it. Last disussion with Doc in Aug said lets see in Jan after MRI. The inflamation is what really concerns me
actually going thru the biopsy is no big deal just annoying. Also prostate is getting bigger each year 50 in 4-18 now 60 in 6-20. Should also add on my second biopsy there was a lot of blood in urine(cleared up in a day) and semen(cleared up in 2wk) but none of this in other 2 biopsy. Also had increased urinary issue after 2nd biopsy

Both a biopsy and an MRI can miss small lesions, which may be high-grade. In addition, the absence of any high-grade lesions is not a guarantee that they won't form in the future. mpMRI and systehatic biopsy are, however, what we currently have. They are essential tools in AS protocols. Repeating them in time greatly lessens the risk that clinically significant will continue to go undetected.
