My MRI results are back and there are 2 'spots'. One is graded at 4 and one is graded at 5 so not good (these are not Gleason scores). One looks like it might be invading the seminal vessel so also not good.

So biopsy is next and we'll see what we will see. At least the biopsy will be guided and they can target those specific areas.

Not the news I wanted obviously but the Doc was confident that it was 'highly treatable' in his words, but I know you don't know what you are dealing with until we get the pathology back.

Hi GM. Even though your appeared to have low-volume G6 disease in 2014. AS requires regular, active monitoring with biopsies and imaging. Your PSA went up 2, 3, and then 4X from '14 to '18, let alone the recent dramatic rise. I assume your docs were pushing for biopsies during those years. It is true that PIRADS 4 and 5 lesions sometimes prove to be benign, but you should be prepared to treat -- based on your biopsy results, of course.

A reminder to ensure your doc is planning on a targeted + systematic biopsy rather than targeted only.

Hope you do well with this and it ain't over until the fat lady sings.

I had a similar MRI with a PIRADS 4 lesion and a PIRADS 5 lesion prior to a guided biopsy and you can see how my story unfolded. I got a copy of my MRI images and the PIRADS 5 was clearly defined and the PIRADS 4 could only be seen by a radiologist, not normal people. Did the standard 12 cores plus one directed at each lesion and waited for the phone call. Little bit of anxiety starts to creep in after the MRI but you know the drill by now.

Hi GM...sounds like a followup biopsy is long overdue. A targeted biopsy should answer a lot of questions.
