I am not sure what you are asking, your information strongly indicates a biopsy is needed. I might suggest going to the place that offers a transperineal biopsy if you need a deciding factor. You also want to be with a doctor you trust. Get the biopsy at the best place for that and return to the doctor you prefer for interpretation and guidance. Good doctors can be cocky at times, a mixed bag to deal with.

I do not think that a biopsy will change anything to do with EPE and any other parts of the cancer if found. They are just part of the process like a DRE that no one likes but does them for the greater good.

Debralyn, this video must be old. The Dr mentioned in the video is no longer at Mt Sinai, since he ran afoul of Medicare rules, allegedly "performing" up to 9 surgeries at the same time, but in fact having residents doing much of the surgery. He no longer takes Medicare, (lol) according to his website, I'm guessing because Medicare no longer wished him to.
I'd say more, but i would run afoul of the forum rules, but you can google if interested.

My husband just had surgery Friday. I did all the due diligence from biopsy to surgery. Along the way, I came across a series of videos called "Mark's Prostate Cancer Experience."

This fellow, Mark, found one of the nation's best urology-surgeons in NYC. This surgeon has an entire floor of his own at Lenox Hill. I don't know where you are in NJ, but this might be worth looking into.

Mark recently posted in his Facebook group that his prostate cancer has recurred, and that he is planning on salvage radiation treatments.
