I believe you answered my concern. I was curious if Decipher score was useful in predicting local occurrence risk as well as metastasis risk. Since my post surgery biopsy showed both EPE+ and SM+ occurrence is always in the back of my mind. PSA test next month will be exactly two years since my first post surgery PSA test. about one month away from testing the anxiety starts to build. 😟 Thanks for the info!

As to whether surgery option isn't appropriate for someone 76 years old, I trust my age and physical health have been considered already by my surgeon, but I will ask him directly. I'm guessing that the severity of side effects could be worse the older you are.

I think that if the patient is otherwise in good general health, he'd be OK'd for a RP at 76.

I was 66 at surgery time, 71 now, and have no idea how keen I would be about surgery at 76, it is not a hip replacement.

I also had radiation and it was easier. On the other hand, I do not have any prostate related urinary issues that I was just starting to experience prior to the event. Radiation did not change that after surgery. Lot of things to consider with these decision. Measure twice, cut once...

I was 69 at surgery and now, approaching 75, I would still do it if I just got my diagnosis--assuming my uro/surgeon would agree. I have a checkup/PSA early next month and will try to remember to ask my doc his policy/experience on upper age for RP, physical-cognitive condition/PE permitting. There have been papers on RP in the elderly if anyone cares to look them up.

I've decided to do EBRT and HT rather than surgery. It's a decision that feels right for me after exploring the alternatives and considering the side effects. I realize there are SE with RT and HT, but the greater possibility of serious incontinence issues with RP versus RT factor in considering quality of life. Thanks to all for sharing their experiences and knowledge with me.

Congrats on reaching a treatment decision, JAW! Usually this brings a feeling of relief after the difficult time we all have after realizing you need to treat your PCa. Best of everything going forward--please keep us posted.

Jaw31, I know deciding between surgery or radiation is a very tough decision, it was for me. Like I said before, now that you have made your decision, don’t look back. I am sure you will have a few questions before you start your treatments. The vets here helped me greatly on what to expect and how to deal with the procedure prep before each treatment. My best to you and hope for a great outcome.

Even though it was 10 years ago, I remember how much I agonized over my treatment decision...and how much better I felt once I made it. When I was first diagnosed, I was on defense, but once I decided on a course of action, it felt like I was playing offense and had a plan of attack. Congratulations on making your decision. I remember the relief I had once I made my decision to with ADT and EBRT. My one year ADT anniversary is coming up next week. I started my EBRT in June and ended in August and have had no urinary issues. Don't second guess yourself now that you've made your decision.

Jim, congratulations on making a decision. And be happy that you studied the alternatives and made an educated decision as that's really important. Good luck; I'm sure there will be a few hiccups but generally your are likely to be cured and on your way to a good life post-treatment.
