Yes you have hyperthyroidism, your TSH is below range , your FT4 is above range.

The next step is to test for the thyroid antibodies , those are TPOab , TGab and TSIab .

Regarding treatment , there are several options, first you need to have thyroid and neck US.

If you have a growing nodules then surgery is the treatment otherwise your Endo might prescribe anti thyroid medications like methimazole, or RAI (Radio active iodine) to kill your thyroid .

I am no expert with RAI but as far as I know some times one need to take RAI pill more than once, our moderator took it and I hope she could shed some light into it .

Are those results from before or after the RAI ? if they are before and you have already taken it three months ago then it is the precise time to recheck ,TSH,FT3 and FT4 .

Shortness of breath can be a symptom of hypo as well as hyper !

What was the interval between those results and the results you posted above ?
You should not be doing blood tests with intervals less than 6 weeks !
That is because TSH is a cumulative measure , and it reaches steady state in six weeks.
That said, your FT4 is below range , and I can only guess that the TSH will start to climb in the near future.
If you start Levo now, you just will become more hyper than you already are , since your TSH will become even lower, I suggest that you repeat the blood work in six weeks !

Levo is Levothyroxine , the generic name of synthroid , it has many brand names, such as Synthroid, Levoxyl,Tirosint, Eltroxin and Euthyrox .

Yes indeed , shortness of breath, chest pain, tachycardia ,arrhythmia , skipping beats and palpitations are all symptoms of hypERthyroidism , and so is dizziness !
