Stool starts in your small intestine. When it gets to the large intestine or colon the fluid starts being absorbed into your body and the stool forms to a more solid mass. The rectum is a very hard place to heal. I had the jpouch surgery 5 years ago and my biggest problem is sores and irritation in the rectum.

The rectom is actually easier to heal than the colon..using rectal meds gets treatment directly to the rectom, which tend to work faster than meds that you take orally and need to reach the colon before they're released. When there's inflammation in the rectom it's referred to as proctitis.

For me the rectal inflamation is actually the hardest to heal. I think that it may vary from one person to the next but for me rectal inflamation is just really stubborn and hangs on and on and on...even when the higher up inflamation is gone my rectum is often still flaring. This causes a lot of bleeding and discomfort for me. Twice the only thing that got my rectum to stop flaring has been a course of pred. This has been very frustrating for me and for my doctor as well.

I'm with thatfield and healing up, I've been on max dose of pentasa supps for 7 months and the urgency and frequency is still ruling my life. As with thatfield only pred seems to get it under control, I've been on 3 short courses in 5 months and now they are talking about
azathioprine to knock it on the head.
We think that the urgency is causing the spasms in my bowel and this is causing the increase in gut motility and as a result not allowing enough of the fluid to be absorbed.
However the rectal meds did clear up the mucous quickly, but still stuggling with urgency and frequecy despite dramatic dietry changes.
Hope this helps.

Yep, sore tum- I totally can relate. It drives me crazy!

starting 30mg, and reduces by 5mg every 5 days.
I have my GI appointment next week, so this time (started again yeaterday) it's only a bridging measure. If I respond appropriately they know it's my immune system that is the issue.
However I also had fistula surgery in July, so potentially it could be sphicter damage or that it hasn't healed completely.
Fingers crossed the pred does the trick this time, that will give them a clear indication of what the next step in my treatment should be.
