My father who is 88 years old is on coumadin. He been on it for a while. Although I've been the only one in the family diagnosed with IBD he just went into the hospital on Sunday due to throwing up blood. When I called and heard he had inflammation in the esophagus I immediately let them know of my recent diagnoses for IBD. Well he started bleeding again and is scheduled for a colonoscopy tomorrow. Hopefully we'll get some answers!
My dad also had a pace maker put in this past May as his heart rate was too low. I'm sure his age has a lot to do with this as everyone starts wearing out at some point in their lives.
I would do some research on coumadin and IBD. I was told it's like taking rat poison and you can only be on it for so long plus with IBD it could cause increase bleeding as your body won't be able to naturally clot. You'll become anemic before bleeding to death...and that's a good thing! You can use that as your warning system.
Research alternatives too. Look like you got a lot going on at once. Try working on one problem at a time if you can. I can relate to your anxiety. I found Mindful Mediation to help calm the nerves.
Personally I'm looking to try Low Dose Naltrexone for my UC. It's an alternative use that supposedly regulates the immune system and doesn't have many side effects plus apparently it doesn't stop working like the other meds. May take some time to work and it may not work at all but it appears to be effective for most and doesn't hurt for me to try. Try searching the forum for LDN posts and google "LDN resources" if your interested. I've also heard it improved your mood...well...some became "annoyed" so it different for everyone.
