Hi,

So I am thinking of taking this plunge (husband as donor)...but have a questions about rectal meds.  I have been on Rowasa nightly for 2.5 years...keeps things at bay, but still not totally in remission.  If I skip a night...problem.  So how bad would it be for doing FT during the day and than use a Rowasa at night.  I know there are the best chances of bacteria grabbing onto non inflamed tissue...so I need the Rowasa to keep inflammation down..but wonder if the medicine will hurt the bacteria.

I'm not sure here. Though I have just started fmt and will remain taking nightly Rowasa. I think it should be okay.

I've never heard anyone mention that mesalamines hurt the bacteria from fecal transplants. I believe I did read somewhere where someone said that steroid enemas might make it difficult for the bacteria to take hold, but I don't know if there's any truth to that or not. I've been doing fecal transplants and have done twelve so far. I'm also trying to squelch a flare, so I do Rowasa, steroid enemas, and fecal transplants once a day. I've never spent so much time on my left side! I have to center my medicine enemas around when my donor (my dad) "goes", so the time of day that I do the enemas vary. I think as long as you do it once a day at some point, you should be good. Good luck! I agree with Meredith I haven't hear that mesalamine hurting the bacteria from fecal transplants / fecal microbiota transplant) either. I took Apriso and canasa mesalamine suppositories while I was doing the fecal transplants and they helped a great deal. Although the biggest breakthrough came from when I added Bupropion and Doxepin to my treatment regiment, so in my case I think Bupropion had an anti-inflammatory effect and controlling IBS diarrhea and muscle spasms was also a very helpful part of what made the FMT work for me.

Still another consideration is swallowing gelatin capsules filled with poop. I know, it may sound like the next level of crazy and I never did it myself, but in two layers of gelatin capsules it won't break apart until it gets to where it needs to go. This could help get it in the upper large intestine or even the small intestine if necessary and therefore reduce the need for as many enemas to get the bacteria to go up the colon.

I can see how you would think that it is a possibility since mesalamine is derived from Sulfasalazine and the original anti-biotics were derived from sulfur and originally anti-microbial properties were some of the suspected reasons for why Sulfasalazine would help.

Another consideration is how how much help oral mesalamine (Apriso, Asacol) or Prednisone tablets would be compared to the enemas or possibly in addition to the enemas.

Thank you all so much for your help.  It sounds like it is safe to use the Rowasa nightly still.  I will def. check out the links later when I have  a few moments (read: kid goes to bed..lol).   Meredith- are you seeing any improvement?

I used to do both at the same time! the feces and then the sfRowasa

Hey, hateuc - sorry, I just saw your question! Yes, I've been seeing a lot of improvement! My diarrhea, blood, and mucus are gone. Granted, I'm using Rowasa enemas, steroid enemas, fecal transplants, sodium butyrate, probiotics, and am pretty strict with my diet, but I believe the fecal transplants are making a difference and will continue to do them until I have my colon out on the 28th.
