My insurance company covers VSL DS #3, however, it was a hassle to get it initially approved. My dr prescribed VSL, and I went to my pharmacy to get it and the pharmacist said the prescription was denied because my insurance company required prior written approval for it. My dr had to contact my insurance company explain why I needed VSL, fax a form to the insurance company. The insurance company reviewed it, and a several weeks later I was able to get the prescription filled at my pharmacy. It's a $45 copay for the VSL, the highest of any of my medications, even though the total cost of the VSL is much less than say the total cost of Lialda.
