6-MP - testing question - high levels of 6-TGN

Hi all,

I searched this question on the site and couldn't find anyone who had this problem. The main problem for most on this drug is low levels of thioguanine nucleotide (6-TGN).

I have been on 6-mp for about a year and a half and doing well. However a recent blood test revealed the opposite problem - high levels of 6-TGN - (1301) which is well over the therapeutic amount, and could be toxic. My doctor has lowered my dose back to 50mg of 6-mp, and we will see if that makes a difference.

Has anyone had this high 6-TGN problem? It seems that I could simply keep decreasing the amount of 6-mp until I am within the right level of 6-TGN?

Actually I have the same problem. I have been on 6mp 75mg for 9 months and my TGN levels are high. I am doing better but still see blood and have 2-3 BM so my GI is doing frequent liver and CBC tests and they are normal so he is not changing dose yet. he is not concern. Acutally he did test 2 times and 2nd time level came down little bit even with out changing does.
But I am concern as I have joint pain and swelling.Which he says goes with colitis. I am schedule for colonoscopy so after that he may change does or suggest to go on biologics.

6-tgn levels should not be over 400.
Is your toxicity level is range?

My dd used to have low tgn levels.

We added in Allopurinol and were able to come down on the dose of 6-mp and her tgn levels normalized and her toxicity levels came way down.

Ask your Dr if adding Allopurinol may be an option?

I jusr got back my results testing 6mp and it says My number is at 14. There was no suggested average as to if that normal or not. I looked online and didn't find any info. Waiting for the doc to send me an email about the results. Any suggestions in the meantime?

I'd wait for your gastroenterologist to get back to you. 14 could mean anything, without knowing the high and liw accepted range...
