I think some severe cases ended with the death of patient, before modern meds (anemia, dehydration, starvation, etc). Colectomy was possible and the only option before modern meds (if you go 50 or so back). Yeah people go in and out of remission, but some flares and symptoms were severe then, as they are now. Iologics are less than a decade old for uc, if it wasn't for biologics I would have needed surgery, for sure.
Of course, but severe cases necessitating surgery only account for about
20% of the UC population. Most are in the mild to moderate categories.
Biologics don't always work. Having Remicade was the equivalent of having saline infusions for me. In fact the surgery rates for IBD don't really seem to have gone down at all - at least not so far.
