Does anyone get these before doctor's appointments? I am sick of filling them out and wasting paper and time. I don't see any point in these thing...

I've had them on occasion with specialists such as surgeons, mammograms, etc. Never during regular appointments.

Are yours usually for insurance purposes? Ask why they need them.

HUGE pet peeve of mine. I have to go for periodic iron infusions. This is an ongoing thing probably forever so I'm at my hematologist every 3 or 4 months. They have me fill out the SAME paperwork every time. Really?? So annoying.

They should just make the form so you can fill it out electronically. it's a pointless form but at least if I didn't have to waste printer ink and paper, fill it out and bring it with me, i would consider doing it. It wouldn't cost much for them to activate the form either, i don't understand why they can't figure it out.

Yes... I think the faulty and divided medical system lends itself to tons of meaningless paperwork here. Wish there was a way to centralize everyone's health information so nurses and doctors would be able to pull it up with a simple search (like ssn number).

There's tons of ground to be made.

i usually have about a 12 page packet to fill out every time i go in. most of the core stuff is the same, but sometimes there are different questions. since my doctor is at a research hospital, i always fill out the study papers. he is doing one right now on the effects of IBD on the autonomic nervous system. there was also one about flexibility and IBD. he had my doing certain stretches, touching my toes, touching my hands behind my back. i actually don't mind filling out the paperwork just because there's always a surprise.

My paperwork is never interesting. it's like a little smile on one side and a frown face on the other and it was say How hopeless did you feel over the past whatever # of weeks and you circle a number between the happy and sad face. I don't think it's very effective for determining anything :) If they want me to fill it out they are just going to have to give it to me in the waiting room. I am not going to spend time on it here, I have too many other things to do and it's a waste of ink and paper.

I think it's funny how more and more stuff is electronic now. They're so big on this HIPPA stuff and yet they verbally tell you a bunch of stuff and then have you sign on an electronic pad and you have absolutely no idea of what you are actually signing until they print it out after the fact. You could literally be signing anything and it might not even be connected to what they just told you (for all we know).

Yeah, a pointless questionnaire before each remicade infusion, every two months. I laugh at the "are you pregnant question", since I'm a guy, so I hope the heck not...

For a regular GI appointment, the nurse always runs through a bunch of oral questions while weighing me and taking my blood pressure (any new symptoms, new conditions, new medications, are you in any pain today, etc). Better than a written form, she checks boxes in the computer; more efficient and less wasteful.

Have had to fill out questionnaires for mental health stuff, but never for anything Crohn's-related. The only thing that's done before a routine GI appointment is a weigh-in, and even that I don't have: I just politely tell the nurse I don't want to be weighed. (It's different if they need to assess drug dosage of course, but I'm not on any Crohn's medication and hope not to be for a long time.)

I was supposed to fill one of those out for my physical with GP in November. I handed the blank form to her and asked if she really needed me to fill out the form. She laughed and said no, but we are friends and knows me pretty well. The only things we went over were mental health/stress questions.
I really hate them.

If yours says 'How hopeless did you feel over the past whatever # of weeks' it sounds like the ones I had to do every time I went for a counselling session. I was seeing a counsellor for depression, it was nothing to do with UC.

I've never had one for anything else. Maybe it's a new thing they're bringing in?

I was a bit surprised the other day when my GP asked me if I had any children. I was like "Don't you know the answer to that?!"

Bearing in mind that I've been with the same doctors since I was little, and that even if they had amnesia, they have all my medical records on file! lol!
