Wow. I say it every time but I find it so incredible that doctors will move on to harsher stuff (in this case prednisone) without even trying mesalamine enemas.
I suggest you need to study up on UC because it sounds like you might not even understand what asacol and lialda are. Asacol/lialda/rowasa are the same thing: mesalamine, the primary drug of choice for UC. Asacol/lialda are oral mesalamine pills, while rowasa is rectal mesalamine enema. Both drugs are trying to topically treat the end of the colon.
By taking mesalamine in enema form, you are directly squirting the medication right where it needs to go. With pills, even though they are designed with a special coating to dissolve in the colon, you are still crossing your fingers hoping they survive the entire digestive tract but then dissolve right at the end. Which do you think sounds more effective?
I agree with others here, you really need to try rowasa (mesalamine enema). If you've had UC for 3 years without mesalamine working, and you've been actively flaring for 4 months, then it's probably also a good idea to try a steroid enema at the same time. Again, same story: the enema will put the medicine right where it needs to go. As a bonus, a steroid enema will have fewer side effects than systemic oral prednisone. As far as oral mesalamine goes, I'll just give you my story: lialda never worked for me, rowasa worked instantly and completely, so I was able to fully stop lialda without a problem.
