Suppositories: Pentasa v Asacol

The Asacol suppositories are 500mg and Pentasa 1g. I probably need to take 1g at night; however the Pentasa ones seem to be softer and want to come out quicker. How can the same medicine be so different?

I'm thinking of switching to two Asacol suppositories instead.


Anyone else had issues of the same medication behaving differently under different brand names?

I initially had Pentasa suppositories when first diagnosed with protitis. I found them really hard (they looked just like the oral tablets) and not easy to insert. Unfortunately, even though they worked initially, I stopped them because They caused a fissure and then I had a bad reatction and they made me worse. I also have taken oral pentasa and they didn't agree with me either.

Asacol worked great. I have used two together in the past just to make the dose up. They are softer, easier to insert and worked really well for me. I much prefer these.
Now that my inflammation reaches higher I find salofalk enemas wonderful. I use them for a long period of time because healing is slow and they don't work overnight. You have got to remember this when using these meds. I used them for about four months after my symptoms had subsided and then gradually reduced them to once every three days.
Give your meds chance to work. I am a very anxious person like your self and expect immediate responses. You get in that rut where you will try anything but I have to keep reminding my self that things don't happen overnight. Yesterday was a bad day for me with multiple bathroom trips but today I have only been twice. Small Steps.

I am just going by what what my GI says (and Quincy!) to be honest. My doc wants me on 3g Mesalizine Oral, and 1g Enema OR suppository. It was the suppository that worked really well for me 2 monhts ago and I shouldn't have stopped it to start the Enema; and further intervention by the steroid hemmie treatment proved that the problem is in the rectal area.

So I will continue on the 1g suppositories indefinitely and take the Enema maybe every two days.

I just wondered why the same medication is different with two different companies and as my doc says, these suppositories can make the hemmies worse.

I think even though the active ingredient is the same there are other ingredients in there that may make a difference. The suppositories and enemas may make hemmies worse due to trauma when inserting them. I use KY jelly to lubricate things when inserting the enema tube. It really helps a lot.

What you gi says sounds good. Stick with it and good luck.

I can't imagine 500mg being all that effective. I've always used 1g Canasa or 4g mesalamine enema, and I had good luck for a while, eventually it became almost ineffective for me. I used to use hot water on the tip of the suppository to soften it. That makes it much easier to insert. I think that was a quincy tip.

It would depend on what other ingredients are in the suppositories. Some melt faster...

I don't see that you'd have to stay on them indefinitely....it's not necessarily all or nothing. When you talk with your new doctor in the US...things may change.

I have an itchy anus and can feel something sharp down there, quite clearly I have hemmies which are playing up - giving me the sensation to go - and inadvertently growing my anxiety and chances of a flare up. I think I may have to do the "no looking" policy for 2 weeks and then see the GI.

Use the ointment externally and put a dot of it into the anus just a bit.

Keep us posted...obviously, you can get in to see your GI quickly.

No more ointments and piles treatments - I have had enough - there is only so much a man can stick up his butt! I am going to treat the Colitis and hopefully that will make me go less; which in turn will clear up the hemmies. I will take more fibre, drink more water, treat colitis and then get the Hemmies seen in America!

I'll bet that attitude will last for a few days and you'll use the ointment.

What kind of fibre exactly?

I will have more Fibre food - porridge, vegetables - they don't cause me diarrhoea. My biggest issue is I don't drink much water at all - I will do that more.

And I won't use ointment, I have been doing so for 6 months and it's making my anus thin!! I need to give it a break for a few weeks until I see a new GI.

I thought you just got the Proctocedyl 2 weeks ago?

You could add fibre supplements....
Upping the water to 6 - 8 glasses a day is good.

Hope things improve to give you a nice break.

I've used:

- Proctosedyl
- Germaloids
- Preperation H
- H Cream

All on and off for 4 months. I just had a BM and I am so sore down there that it almost feels like a fissure.

I've had that the past month or 2, GI confirmed it was a fissure 3 weeks ago, my UC had calmed down so she took me off the pentasa suppositries(still on the 4mg granules), i take a stool softener ever day and some probiotics+yogurt, it still hurts a bit after a poop but not as much.

My Colitis has calmed down too in that I don't have loose stools or mucus which is the usual tell tale sign. However. I have minor specks of blood every few days; and massive irritation with the suppositories and the enema. Maybe I should do as you have done which is stop the rectal medication (which have helped a lot) and just use the granules.

Do you have specks of blood Chris?

Not a few months now thank god(pretty much a day or 2 after being on the suppositories it stopped), down to 2 farely solid bowel movements a day(morning and after dinner in the evening), lot of skidmarks due to the fissure, butt is a bit tender after a poop but nowhere near what it was when i first started the suppositories. They really are not a nice pill to put up there, was so glad when she said i can stop taking them.

So you get the odd skid mark of blood but no real streaks or pools of blood? MIne is very similar and mine also stopped as soon as I started the supps... now they are irritating my hemmies. I am going to see the GI tonight.

Doc wants to stop all rectals for a week and see what happens as he thinks that maybe the Mesalazine is causing issues topically. I'm so confused. But all will be revealed next week's Sigmoidoscopy.
