UC has a tendency to spread in extent and severity over time. Proctitis is limited to just the rectum. UC always includes rectal involvement, but can spread upward to include the sigmoid colon, transverse colon, and keep going until all of the way up to cover the entire large intestine (known as a pancolitis). For that reason, some proctitis patients take an oral mesalamine to help reduce the odds of inflammation spreading upward.
Yes, some proctitis patients do take oral prednisone. Certainly try rectal-route steroids first (which you did with steroid suppositories), however they are limited in strength. Oral prednisone is a much. much stronger medication that could help.
Before going to oral prednisone, you could try a steroid enema as they are stronger dosages than suppositories are.
